We may not be able to respond to requests for disclosure, etc. if the individual concerned has refused to provide personal information. We assume no liability whatsoever for loss or damage (including lost profits) or detriment suffered by the individual concerned resulting thereof.

If you contact us, we will send you a Personal Information Disclosure Request form by post, fax, or email, etc. Please fill out the form and return it to us by post enclosing the necessary documents. (Postage to be borne by the sender) After verifying the identity of the individual concerned (or his/her representative) we will respond to the request in writing.

We may not be able to respond to requests for disclosure, etc. if the individual concerned has refused to provide personal information. We assume no liability whatsoever for loss or damage (including lost profits) or detriment suffered by the individual concerned resulting thereof.
